Prostate Cancer Surgeon in Delhi

Prostate is a small walnut sized accessory sexual gland located at the base of the bladder. With advancing age there is enlargement of this gland. This enlargement is most often benign . However it can also be a malignant enlargement called Prostate Cancer.

The main predisposing factors are racial and familial predisposition. While by and large people in the Indian subcontinent have a low incidence of prostate cancer, this is fairly common amongst people of Africa, the Americas, Australia and South Pacific Islands. Most often the cancer is slow growing and causes little harm. Some are however life threatening and can rapidly spread and debilitate the patient.

The diagnosis is based on a detailed history and physical examination. After this usually the Serum PSA blood test would be done and repeated to confirm an abnormal reading. If there is a possibility of a cancer, then usually a Multi Parameter Prostate MRI scan will be done. This scan clearly shows where a tumor is likely to be located, where all it may have spread and possibly how aggressive the disease may be. The next step would be to do a Trans Rectal Ultrasound guided (TRUS) biopsy of the prostate. With this a diagnosis of prostate cancer is established.

The next step would then be to see the extent of spread of the disease. Most often this would need a PSMA PET scan or Bone scan.

The management of the cancer then is decided depending on the type of cancer, the stage of the disease, the grade of cancer and the physical fitness of the patient.

In early cancers confined to the prostate

In young patients Radical Prostatectomy by Robotic surgery or in selected patients surveillance may be the answer. The operation of Radical Prostatectomy is removal of the entire gland. This has the big advantage of removal of the entire cancerous growth and at the same time the best chance of preservation of continence of urination and preservation of sexual function. In the Surveillance program in few younger patients , the cancer is kept under watch for a few months or years until it is then mandatory to intervene by Robotic Radical Prostatectomy or Radiation. This allows a person to continue maintaining the quality of life for a long time.

In early cancer for the older patients

Ususally the treatment would include a combination of Radiation and Hormone therapy. The radiation plan would be decided by the Radiation Oncologist and then delivered over a few days.

In patients with advanced metastatic Prostate cancer , the treatment will always include a combination of Hormone Therapy, Chemotherapy along with Radiation to sites of bony pains.
